Can you bill my Insurance??

Answer:

Yes, we can bill your insurance providing it is a covered item. Some insurance companies require us to obtain prior authorization for covered services. We will always verify your insurance and inform you of any out of pocket cost to you prior to proceeding to order or fabricate.

Do I need a prescription??

Answer:

We do need an initial prescription to provide services. However, we can schedule an appointment to evaluate without one. Your insurance will require a prescription along with documentation form your physician.

Do you offer payment plans??

Answer:

Yes, if you have a substantial amount that you will be responsible to pay we can definitely work a monthly payment plan. If the services you are receiving are not covered by your insurance we require at least 1/2 down prior to ordering or starting any fabrication. Usually the remaining balance is required at time of delivery.

Does insurance cover ________________??

Answer:

Most services we provide are covered by most insurance companies. We will always verify your insurance and inform you of coverage and any out of pocket expense prior to providing anything. There are some services that are excluded from all policies, while others require specific documentation prior to your appointment.

Does it cost me anything for an evaluation??

Answer:

There is no specific office visit charge. All charges that are incurred include office visits, fitting, adjustments and products provided.

How long do I need to wait after my mastectomy to be fitted for breast prosthesis??

Answer:

There are post-surgical products that you can get to wear immediately after surgery during healing process. For the initial fitting of the breast prosthesis and mastectomy bras, you need to be completely healed, this usually takes approximately 4-6 weeks.

What do I need for DM Shoes??

Answer:

Prior to scheduling an appointment we need to make sure you qualify for therapeutic shoes. There are forms that the physician who treats you for your diabetes needs to complete and document in your chart. Most physicians may have you schedule an appointment if you hadn't had your diabetic management follow up recently. Just because you are a diabetic does not mean your insurance will cover the shoes. Once you have these forms completed we can schedule you an appointment to have you evaluated with our Orthotist. Please see the Patient Info Tab above for all forms that will be needed.

Why do I need to schedule an appointment??

Answer:

Although sometimes we can see walk-ins, we do schedule appointments to ensure you are properly fitted. A designated appointment will ensure you have time set aside to be properly measured, fitted, etc. Should you require a service that needs to be custom made or ordered, we would take measurements/cast then fabricate or order. Once everything is completed and or received we would contact you for an appointment for a final fitting and delivery. Should you need any adjustments to a service we previously provided, please contact us and we can schedule you another appointment to get it taken care of.
